To provide acomprehensive accommodation based support service to vulnerableadults and young people living in the singles hostels which formpart of Camden s Adult and Young People s Pathway ensuring thatthey are enabled to develop the skills and abilities to movethrough the pathway to independent and settled accommodation.Service users present with a range of support needs includingmental health drug and alcohol domestic violence and offendingbehaviour.
1) To carry out regular assessments and reviews of needs on acasework basis conduct key work sessions and develop support plansto identify actively and enable service users to secure appropriatesupport from partners including physical and mental health servicesdrug and alcohol services and employment training and resettlementservices. Support needs and actions to be agreed where possiblewith service users and other key stakeholders. 2) To provide a highquality and responsive accommodation based support service in orderto assist service users to develop the personal and social skillsthat will enable them to maintain their accommodation improve theirhealth and wellbeing improve their employability and financialinclusion and decrease any negative impact of their behaviourwithin on theirlocality.
3)To carry out regular and comprehensive assessments of riskassociated with service users including Safeguarding and to ensurethat identified risks are actioned and recorded in accordance withservice policy and procedure and Commissioning requirements. 4) Tosupport service users to move positively through the Pathway inaccordance with the principles and requirements of the HostelPathway Model and promote housing options which will includeprivate renting and shared accommodation. 5) To develop andmaintain positive working relationships and liaison with partnersand stakeholders to include convening/attending professionalsmeetings case conferences and handover meetings whereappropriate.
Experience of providing accommdation based or floating support tovulnerable people with complex needs. Knowledge of housing optionsavailable to vulnerable single people. Knowledge of the welfarebenefits systems and legislation. The ability to carry outassessments of need and risk and identify appropriate responses.Effective oral and written communication that can be adapted tocommunicate with a diverse and challenging clientgroup.
