Responsibilities:
Provide emotional support and counseling to patients and their families.
Develop and review treatment plans in collaboration with the healthcare team and participate in multidisciplinary team meetings to ensure patient care is coordinated.
Coordinate patient discharge planning to ensure a smooth transition from hospital to home or other care facilities.
What we are looking for in a Social Worker:
Social Work England registration
Eligible to work in the UK
Hold a full UK licence
Post-qualified experience
Degree level or equivalent in Social Work
