Key Duties:
Conductingin-depth assessments to determine the needs of adult service users.
Crafting and revising care plans in partnership with service users, their relatives, and other healthcare professionals.
Advocating for service users to ensure their needs and voices are central to care decisions.
Requirements:
A Recognised qualification in Social Work.
Registration with Social Care Wales and DBS
Considerable experience in social work, specifically within a hospital context.

Hold a full UK licence
Post-qualified experience
Degree level or equivalent in Social Work
