An exciting job role has recently become available at the Middlesbrough Metropolitan of Lewisham. They are looking for a new member for their Hospital Discharge Team.
X4 workers required for a 12 month period 2 years post qualifying experience required Request to cover case work/reviews. This will involve the ability to undertake face to face visits x5 days a week with x2 days a week working from an office setting. Learning Disability Team – adults in transition and over 18 with a learning disability

Requirements :
Assessing Needs : Conduct interviews and assessments to understand the needs of individuals and families.
Developing Care Plans : Create and implement care plans to support clients in achieving better outcomes.
Advocacy : Advocate for clients’ rights and access to necessary services.
Collaboration : Work with other professionals in multidisciplinary teams to provide comprehensive support
What we are looking for in a Social Worker:
Social Work England registration
Eligible to work in the UK
Hold a full UK licence
Post-qualified experience
Degree level or equivalent in Social Work
