More about the role:

We are looking for an experienced Registered Social Worker, Nurse or Occupational Therapist to join our professional and dedicated Hospital Discharge team. We provide a 7-day service, operating in a hybrid working pattern with rest days in the week, and are open to providing a job-share system for employees who just want to work weekends. Our goal is to create a strong network of supportive individuals who thrive in a fast-paced environment. This role will suit someone who is eager to get things done and make a real difference in their community.

The role will involve going into hospitals, resident’s homes, nursing homes and residential care homes to maximise people’s independence, with a strong focus on improving their futures. We need people to be flexible, have experience of working in these environments, and approach every day with a ‘can do’ attitude.

Your main responsibility will be to focus on the people you care for and what is right for them. You’ll cover the whole of Central Bedfordshire and out-of-area hospitals, and manage your own caseload, which will include supporting a set number of people through discharge pathways (from hospital and discharge to assess). You will also work closely with the reablement team, who are the first port of call out of hospital.

Our aim is to support people at the most vulnerable point in their lives to get home and recover safely, consequently becoming as independent as possible.

We are looking for the following qualities in the successful candidate:

  • The ability to adapt to different environments and circumstances depending on the person.
  • Motivated and committed to the role.

  • The ability to work under pressure.
  • Be knowledgeable around paying for care.
  • A recognised social work, nursing or Occupational Therapy qualification and be currently registered with Social Work England, Nursing & Midwifery Council or HCPC .
  • Access to a vehicle and hold a full, valid, driving licence.
  • This is a very diverse role where every day is different and presents a new challenge. Some of the areas of work you can expect to be involved in are:

  • Duty function: discharge to assess (going out into the community – visiting residents in their homes, care homes, hospital, etc.).
  • Home recovery service.
  • Care act assessments – working within legislation and latest hospital discharge guidance.
  • Mental capacity act assessments.
  • Discharge planning.
  • As a team, we can offer you excellent career progression opportunities and training should you need it (AMP, BIA). If you already have these, you’ll have the opportunity to use them on a rota basis. We’ll provide you with strong Learning and Development opportunities and support you to undertake CPD qualifications.

Our staff are important to us. We undertake regular staff surveys and encourage a culture of open and honest dialogue as well as a service led ‘thank you’ scheme where we recognise the great work our colleagues do. Our hybrid way of working has led to a lot of positive feedback from our current staff who are able to successfully balance their personal and professional lives.

    What we can offer you

As well as joining a friendly, dedicated, and hardworking team you will have access to numerous benefits, including:
