Homerton Healthcare has a band 6 post to work within the Community Children’s Nursing team which covers the London Borough of Hackney and the City. The Community Children’s Nursing Team offers a 7 day service for, so are you ready to take up this challenge?

The Trust is investing in the team to ensure that those children who can be discharged home early and can receive their care at home can do so 7 days a week. We are looking for a children’s nurse who may have worked within the field of community children’s nursing and wanting to develop their skills further.

The team has both a generic caseload where we have children on the caseload who suffer from life limiting/life threatening conditions to children who require dressing or the completion of their intravenous antibiotics and to those children who have a continuing care package who are on the complex care caseload.

Within Homerton Healthcare, staff are encourage to further their nursing practice through professional development to ensure best practice.

Main duties of the job

Main duties are as follows:

  • To provide specialist nursing care to all children in the community
  •  To provide quality nursing care in accordance with the child’s individualised plan of care as part of the Community Children’s Nursing Team.
  • To facilitate the development of the extended role of the Community Children’s nursing team in conjunction with senior colleagues. This will be demonstrated through teaching, supporting and acting as a resource for all members of the team.
  • To work in close partnership with parents and carers to ensure they are confident and competent in the care of the sick child at home.
  • To manage client caseload and have the ability to work autonomously. To delegate work and support junior staff members according to their knowledge and expertise.
  • To be responsible for the provision of assessment, delivery and evaluation of planned programmes of care.
  • To manage your own time and priorities in collaboration with other members of the Community Children’s Nursing Team.
  • To attend discharge planning meetings within the acute setting s to assist the future nursing needs of patient’s sand provide a seamless care from hospital to the community. To be involved in the providing and receiving delicate and sometimes complex information during these meetings.

    Education / Qualifications

    Essential criteria

  • Register Child Teaching/mentoring 998 or equivalent.
  • Desirable criteria

  • Community Specialist practitioner
  • Skills/ Abilities

    Essential criteria

  • Ability to manage a caseload of children with varying health needs.
  • Ability to communicate complex and highly sensitive information.
  • Ability to link with the acute units as necessary to ensure the smooth transfer from the ward into the home setting.
  • Ability to work within a team and contribute to the smooth running of the team.
  • Experience

    Essential criteria

  • Substantive experience of working with the community.
  • Desirable criteria

  • To have nursed children who have required ventilation.
  • Knowledge

    Essential criteria

  • Knowledge/understanding of confidentiality issues
  • Demonstrate sound knowledge of paediatrics in the community.
  • Clear understanding of policies related to safeguarding.
  • Other Requirements

    Essential criteria

  • To be able to work flexibly across the generic and complex care caseload.
  • To be able to do on call and be an active participant.
  • To be car driver with a clean licence.
