Carry out social work tasks to a satisfactory professional standard in accordance with the policies of the Social Services Committee and the instructions of the Service Unit Head. Comply with and meet the standards set out by the policies and procedures jointly agreed between Social Services and Dorset Healthcare University NHS Foundation Trust.

Account to the Service Manager for the quality of work and provide information on the nature of the work.

To ensure that support plan reviews are carried out in a timely and person centred way evidencing need for continuing support.

Undertake comprehensive assessments.

Contribute to comprehensive risk assessment and risk management plans for adults. 

Undertake care coordinator role for designated caseload in accordance with the standards set out by the policies and procedures by Dorset Healthcare University Foundation Trust.

Undertake carers assessments and work with families. 

Undertake preventative work. 

To provide information and advice to adults and their carers to help people stay healthy, plan for their future needs, use services effectively, take responsibility for their own health and care, and maximise the impact of prevention and recovery services.

To encourage and support people to look after themselves, their families and their communities, promoting choice and control for people and the use of personal budgets and direct payments where possible.

To undertake best interest assessments taking account of the requirements of the Mental Capacity Act.

To assist adults to have realistic expectations about the support and care that the Council is able to offer, through involvement and communication and ensure the most effective and targeted use of resources to meet person centred outcomes.

Act as an Approved Mental Health Professional (or undertake training). Make admissions to hospital under the Mental Health Act.

Act as a Safeguarding Adult Practitioner (or undertake training). 

 Train and act as a Deprivation of Liberties Assessor.

 Act as Care Manager as required. To deliver the service in accordance with statutory responsibilities set out in the NHS and social care legislation and the eligibility criteria for services as agreed by the Council.

Maintain essential records, including information for statistical purposes, and make such records and information available according to quality standards and on core computer systems as required.

 To participate in team meetings and official meetings relevant to the service group, as required; including Multi 

Agency Risk Meetings, Child Protection Meetings, Core Group Meetings, Safeguarding Meetings.

Participate in out-of-office hours standby and statutory functions for which preparation and authorisation has been received.

Participate in and contribute to training.

Manage a part or whole caseload of a specialised nature which may be based on client groups or particular methods of social work and provide recovery focused interventions.

Provide a Team or Service Unit consultancy services as required.

Undertake an assigned development role in the Team or Service Unit.

To engage in and prepare for supervision and appraisal through self evaluation against the council’s values and behaviours; national occupational standards and ensuring that continuous professional development is evidenced.

To prepare reports for court and attend to give evidence in legal proceedings as required.

Perform a designated liaison role as required e.g. dual diagnosis link worker.

To undertake such other duties as may be required from time to time commensurate with the level of the post.

To comply with all decisions, policies and standing orders of the Council and any relevant statutory requirements, including Equality Act, the Health and Safety at Work Act and Data Protection Act.

To maintain registration of SWE where this is a requirement to practice and to undertake statutory and other tasks related to the responsibilities of the service group outlined in the context statement, in accordance with relevant legislation and the Council’s competence framework.
