Proactive Care Team Nurse - Winchester City & South

Job summary

We are looking for an exceptional community based nurseto join our team at Mid Hampshire Healthcare which covers Winchester City & South area. This is a part time role for hours per week (Days to be agreed).

Previous experience of communitynursing is a benefit but not essential as we offer a structured trainingdevelopment programme with formal support, provided. Main duties of the job

As a community nurse, you will be directly involved in facilitating and shaping self-care for patients with long term conditions in order to help keep them as well as possible in their own homes.
