Job overview

The post holder will be part of a dynamic team of clinicians whose responsibility it is to develop and improve services for people over 18 years with common mental health problems in primary care. The role includes working across designated GP practices within the primary care networks, attending weekly integrated care team meetings.

The post holder will provide effective consultation and liaison across the primary and secondary care sectors, whilst facilitating and improving access to appropriate services. The role is to provide mental health screening assessments, risk assessment, medication monitoring, telephone triage, sign posting, referral to other services and brief intervention for people experiencing common mental health problems,

The post holder will also provide support, education and problem-solving advice to other members of the local primary care teams particularly in the areas of assessment and management of mental health problems, working across different geographical settings and undertake home visits.

Main duties of the job

The post holder will undertake specific managerial, administrative, preceptor and clinical supervisor roles as required by the post. To ensure up to date, accurate clinical notes and activity recording relevant to the role and participate in audit and any appropriate approved research.

In response to current local policy and national strategy for mental health, active involvement in developing services to meet the mental health needs of users and carers in primary care, ensuring they are equitable, non-stigmatising and responsive to local need and available to people aged 18 and over experiencing a common mental health problem. To maintain and improve knowledge and skills based on best practice particularly specialist knowledge and skills in mental health issues in primary care. To act as role model and resource for junior/less experienced staff and students from the multi-disciplinary team.

  • •Entry on a Register held by Nursing Midwifery Council (Sub Part 1 RN3, Registered Nurse Mental Health)
  • •Evidence of current continued professional development
  • •Willingness to undertake non-medical prescribing
  • •Post registration experience of working autonomously in community mental health services
  • •Experience of working collaboratively within a multi-disciplinary/ multiagency team
  • •Experience of managing and organising own workload
  • •Skills in assessing and formulating individual intervention plans and outcome measures for adults with mental health problems
  • •Risk assessment / risk management skills
  • •Knowledge of latest evidence based practice and NICE guidance for supporting individuals with Mental Health needs
  • •Knowledge of the principles of clinical governance / safeguarding and its application
  • •Excellent communication skills oral and written
  • •Car Driver

  • •Training relevant to the role - teaching and assessing, clinical supervision, motivational interviewing, solution focused working
  • •Supporting staff in the day-to-day provision of clinical services
  • •Experience of management or leadership
  • •Experience of delivering new and innovative practice in a clinical setting
