An opportunity has arisen for a part time physical health support worker to join a vibrant Early Intervention Service on a permanent contract.. 

The team covers the South and Central area of Manchester and operates from Chorlton House. 

The post holder will work alongside the Early Intervention Team covering the South and Central area of Manchester under the supervision of the physical health lead and team manager, undertaking physical health checks, physical health clinics and general health and wellbeing assessments. The post holder will assess physiological observations and promote healthy lifestyles for our service users. The post holder will assist in enabling service users to access the support they need to recover from their mental health and physical health problems, promoting recovery in order to return to everyday living. They will also assist care co-coordinators in working across boundaries of care to plan, implement and evaluate care within care pathways as part of the stepped care model.

Main duties of the job

Detailed job description and main responsibilities

  • The post holder will partake with the physiological assessment of our service users. They will take and record their blood pressure, pulse, weight, BMI, venepuncture for required blood tests
  • ·Assessing service users holistically. Assessing lifestyles – exercise and activities, eating and drinking, smoking, alcohol and drug use and giving advice on healthy lifestyles and signposting our service users to other services.
  • The post holder will work alongside the physical health nurse in clinics and community visits but will be participate in team working.
  • The post holder will work with other team members to ensure the team operates a holistic and multi-disciplinary approach
  • The post holder will contribute to the CPA process – liaising with the care coordinators in view of contributing towards CPA needs and reviews

    Qualifications

    Essential criteria

  • Recognised competency based level 3 NVQ in relevant subject area or equivalent demonstrable experience
  • Experience

    Essential criteria

  • Experience of working in MH care setting
  • Experience working in a community setting
  • Experience of MDT working
  • Skills

    Essential criteria

  • Excellent written and oral communication skills
  • Good organisational skills and ability to work flexibly
  • Venepuncture trained
  • Desirable criteria

  • Understanding of how to engage service users
  • understanding of health promotion
  • understanding of physical health needs for MH service users
  • skills in physical healthy care observations
