Job summary

We have an exciting opportunity for a clinical pharmacist to joint our incredibly friendly practice, based in Oxfordshire.

You will be working solely for Hedena Health and join our existing team of 3 Pharmacists. As a practice we are innovative and forward thinking and determined to deliver the best care to our patients. You will be supervised regularly and continuously supported by our senior clinical pharmacist and GP line manager.

Main duties of the job

Thepharmacist will work as part of the multidisciplinary team in

Patientfacing roles - assisting with medication related problems and queries

Stafffacing roles - providing support to general practice staff in relation torepeat prescriptions and medication queries

Prescribingteam roles- working with the prescribing team on safety and quality of thepractice prescribing

Everymonth each pharmacist will have blocked time for supervision provided by thesenior clinical pharmacist

Day-to-daysupervision will be provided by the senior clinical pharmacist or thepractitioner leading the acute hub (if pharmacist is trained in minor illness)

Additionalsupport and training provided by GP line manager

Job description

Job responsibilities

Theworking day will be 9 hours, with 30 minutes for lunch as well as 45 minutesfor coffee (30 minutes AM session and 15 minutes PM session). The morning willbe from 08:30 to and the afternoon from to

Atypical day (when all pharmacists are in) will be structured as follows:

1sthalf - Two hours blocked for dealing with repeat prescription requests (wherethe admin team have been unable to issue the prescriptions due to alerts orbecause the medication is out of date/authorisations). The rest of the sessioninvolves routine appointments ( pre-booked medication reviews) or reviewingblood test results (subject to training blood tests ordered by thepharmacists for DOAC/DMARD monitoring or diabetes reviews)

2ndhalf Routine appointments for pre-booked medication reviews. Dependingon experience and skill set, sessions may be allocated solely for chronicdisease reviews diabetes. One pharmacist is allocated an entire session towork off the triage list (subject to training) actioning quick wins or urgenttasks and allocating routine appointments where necessary ( for routinemedication reviews which cannot be done from remote review of notes)

Onesession a week blocked for contracts related work. Admin sessions also providedto work on audits.

If trained in minor illness pharmacists may beallocated sessions to work on the acute list seeing or calling patients withacute illnesses seeking same day medical attention. Sessions supervised by theemergency practitioner leading the session. There will also be ad hoc meetings with the prescribing lead on practice prescribing issues to be agreed in advance.

Thepharmacist will work as part of the multidisciplinary team in -

Assisting patients with medication related problems andqueries

Providing support to general practice staff in relation torepeat prescriptions and medication queries

Working with the prescribing team on safety and quality ofthe practice prescribing

Medicines reconciliation including requests for newmedication on ( on discharge summaries/outpatient letters) and reviewingrepeat medications for newly registered patients

Support with completing necessary training such asindependent prescribing if not already qualified

Scope to further develop skills in areas of interest respiratory, diabetes, minor illness

Carry out structured medication reviews for all patientcohorts

Undertake clinical audit to support patient care and safety

Participate in contracts related work helping the practiceto achieve targets for QOF, IIF and the prescribing incentive scheme

Assist with chronic disease diagnosis, management andreviews ( hypertension, diabetes, CKD, hypercholesterolaemia) subject toexperience and training

Work allocated toour pharmacy team is added to a dedicated triage list. Our pharmacists have aprotected session to review this list which gives them the autonomy to controlwhich patients are booked into their routine appointments and allows them adequatetime to action any urgent tasks.
