- Working hours: 9am – 6pm (Monday to Sunday)
- Weekend work available but optional
- Hourly rate range from £21-27

Home First Therapy Team completing home visits for patients on the day of discharge from acute hospital to assess patient health and social care needs in own environment, following the discharge to assess model.
Requirements:
- HCPC registration
- Previous experience essential
- Right to Work in the UK
