Within thecontext of selfdirected support personalisation and working as amember of a multidisciplinary team responsible for meeting theassessment care management and support needs of vulnerable adultsthe post holder will:

  • Managean independent caseload assessing people with physicaldisabilities/ sensory impairment/ mental ill health/ learningdisabilities.
  • Takespecific responsibility for the provision of appropriate equipment/ minor and major adaptations / assistive technology and theplanning/implementation of appropriate reablementprogrammes.




Able to demonstrate an understanding of the role of an occupationaltherapist in a community setting. Able to demonstrate anunderstanding of disability issues. Ability to communicate andengage successfully with citizens and their family / carers.Ability to work effectively as a member of a multi skilled team.Ability to manage own time and workload. Ability to communicatesuccessfully within the organisation and with a range of externalorganisations. Ability to record work in a timely manner, preparereports as required and maintain clear and concise case notes.Basic IT skills as required within the context of this role.Commitment to learning and development. Able to demonstrate anawareness of health and safety issues. The ability to communicateverbally with customers and provide advice and/or information inaccurate spoken English is essential for the post. Demonstrate anunderstanding of the needs of vulnerable adults and their family /carers. Demonstrate an understanding of the concept ofpersonalisation and self-directed support to enable citizens tolive independent lives exercising choice and control in decisionmaking working in a reabling manner. Demonstrate an understandingof person-centred support planning, the importance of reablement& how equipment & adaptation provision can contributetowards this process. Demonstrate a broad understanding of therange of available resources to support citizens to maintain theirindependence. Understanding of the importance of signposting touniversal services. Demonstrate an understanding of the need torespond proactively to customer feedback including learning fromcomplaints and compliments. Demonstrate a broad understanding ofthe Core Principles of the Mental Capacity Act 2005. Demonstratesound knowledge and understanding of the Care Act2014.
