Job summary

This position will be based in one of our 2 North Wokingham localities. The working hours are hours per week and the working week covers 7 days with shifts between 7 am and 7 pm.

The role of the Nursing Associate/Assistant Practitioner is fundamental to the success of Community Nursing Teams. The post holder must be able to demonstrate the competencies required to perform role and role model the Trust's core values well as exhibit the behaviours required as part of those values.

Main duties of the job

The Band 4 will work across the healthcare teams. He/she will provide planned care with proximal supervision from a registered practitioner to deliver a seamless service to patients.

The Band 4 will work with other team members embracing case management and the long-term conditions and Aging Well Agenda thus preventing avoidable hospital admissions and facilitating appropriate earlier discharges into the community.

He/she will have excellent communication and observation skills. This role requires many skills, these are just some of the tasks you will be required to complete:

  • Venepuncture
  • Wound care/Compression/Dopplers
  • Insulin Administration/Other Injections
  • Catheter Care - including replacement
  • Holistic Assessments
  • Raising Safeguarding Concerns
  • And much more..

    You will be required to use an iPad/laptop for documenting patient contacts. He/she will work within defined protocols and be able to take appropriate action to deliver planned care.

    Full training will be given to the successful candidate.

  • Educated to GCSE standard in English and Maths
  • Nursing Associate Qualification or Assistant Practitioner Qualification
  • Previous Experience

  • 2 years previous experience within a community setting

  • Able to work flexibly across service area to enhance quality of service provision.
  • Full Valid UK driving license and vehicle available for use for work travelling with confidence to drive during unsocial hours and in unusual environments with use of care for work purposes.
  • Able to work bank holidays, weekends and unsocial hours to meet service needs (shifts from 7 am - 7pm)
  • Able to work in challenging environments.
