The pharmacy team at South Tees Hospitals NHS Foundation Trust have an exciting opportunity for an Advanced Clinical Pharmacist in Pain and Palliative Care as a fixed term part time contract as maternity leave cover. This role forms part of our progress towards advanced level of practice for pharmacists.

The successful candidate will work collaboratively as part of the respective multidisciplinary teams to provide specialist medicines optimisation, outstanding leadership and innovative service development and improvement. The role will encompass delivery of safe, efficient and cost-effective pain and palliative care services to patients within South Tees NHS Foundation Trust and across the wider integrated care system linking secondary care with primary care and regional networks.

They will be keen to take on a new challenge and able to work using their own initiative to innovatively balance and manage the requirements of the specialist pain, palliative care and hospice teams, patients, carers and other stakeholders.

Main duties of the job

  • To be responsible for leading, delivering and evaluating clinical pharmacy and medicines optimisation services across the specialist pain and palliative care teams.
  • Ensuring optimum medicines management within pain and palliative care services, which are provided in accordance with relevant professional, ethical, legal and locally agreed policies, procedures, guidelines and standards.

  • Providing a direct clinical pharmacy service to Teesside Hospice 1 session (4 hours) per week.

  • Providing specialist pharmacist advice, guidance, and information on treatment choices across the multidisciplinary teams in pain, palliative care, and Teesside Hospice.

  • Attend and actively participate in specialist ward rounds, clinic settings and other patient reviews.

  • Provide education & training across pharmacy and the multidisciplinary teams within pain and palliative care.

You will be expected to attend the leadership development programme, and the Quality Improvement Programme that the Trust delivers. This programme aims to:

  • Practitioner level in quality improvement, equipping you with the skills to champion, lead and complete quality improvement within your role
  • After you complete this four and a half days of training you will be able to explore further leadership and improvement training opportunities, we offer further in-house courses dependent upon your role and bitesize programmes and leadership apprenticeships at level 3,5 and 7.

    Experience

    Essential criteria

  • Wealth of experience in clinical pharmacy within a hospital setting
  • Experience of providing directorate level pharmacist services budgetary reports, drug usage reviews
  • Developments of protocols and policies
  • Experience of developing strong professional relationships with other healthcare professionals
  • Experience of leading and contributing to relevant research, audit and service improvement projects
  • Experience of involvement in multidisciplinary service development
  • Desirable criteria

  • Staff leadership and management experience
  • Experience in mentorship and training for other staff
  • Teaching experience in clinical and classroom setting
  • Qualifications

    Essential criteria

  • Masters Degree in Pharmacy or equivalent
  • Registered Pharmacist with the General Pharmaceutical Council
  • Diploma in Clinical Pharmacy or equivalent relevant postgraduate clinical qualification
  • Pharmacist independent prescribing accreditation or actively working towards qualification
  • Evidence of CPD and ongoing education
  • Desirable criteria

  • Relevant postgraduate clinical qualification
  • Membership with Royal Pharmaceutical Society (RPS)
  • Working towards RPS advanced clinical practice or consultancy frameworks
  • Knowledge and skills

    Essential criteria

  • Ability to apply theoretical knowledge to clinical practice
  • Involvement in development of evidence-based clinical guidelines, polices and protocols
  • Strong knowledge of medicines information resources
  • Desirable criteria

  • Membership of regional/national/international specialist interest groups relevant to field of practice
  • Disclosure and Barring Checks (DBS)

The method of payment for this is via salary deduction from your first month's pay.
