J-pouch and IBD surgery Clinical Nurse Specialist, Band 7

An exciting opportunity has arisen for a dynamic and motivated individual to join the IBD and j-pouch surgical team at Chelsea and Westminster hospital. We are looking for a nurse with experience in bowel surgery to provide specialist nursing support and advice to patients undergoing surgery for IBD and j-pouch formation. You would work closely with all members of the MDT to provide comprehensive follow-up, in nurse-led clinics and joint clinics with focus on multidisciplinary management of Crohn's disease and Ulcerative Colitis.

The established IBD and ileoanal pouch Surgery Centre offers a dedicated "one-stop" ileoanal pouch clinic, with more than 25 major abdominal pouch procedures performed every year. We also perform complex surgery for primary and recurrent Crohn's disease, including strictureplasties and Kono-s anastomosis. You would be involved with the continued development of the service, contributing to teaching programmes, research, audit and patient reported outcome measures data collection.

Main duties of the job

The post holder will develop and provide an advisory and supportive service to all patients undergoing IBD surgery and ileoanal pouch surgery. They will provide education and information to patients considering surgery and to those who have undergone emergency surgery. As part of the team, they will work to develop the IBD surgery service.

They will provide long term follow-up, following a structured pathway and by monitoring email and telephone contacts, liaising effectively with all members of the multidisciplinary team, providing continuity of care into the community and throughout the patient's rehabilitation.

They will act as an expert resource to professionals involved in the care of these patients both within the acute and primary care setting.

The post holder will maintain oversight of the services activity, governance and clinical outcomes, working with the team on change initiatives as required.

Job description

Job responsibilities

Clinical Practice

  • To meet with elective patients pre-operatively to give information and education regarding proposed surgery.
  • To provide post-operative guidance, support and advice, enabling a safe discharge and liaising with community services as required.
  • To provide long term follow-up, according to the patient pathway, by telephone and face to face contacts within the IBD and ileoanal pouch clinic.
  • To be a point-of-contact for patients, monitoring emails and phone calls, liaising with the MDT as needed.
  • To maintain the IBD and ileoanal pouch database.
  • To write and maintain literature, supporting the information and education given to patients
  • To attend the cross-site IBD MDT and the monthly ileoanal MDT.
  • To participate in joint IBD surgery ward round.
  • To support the expansion of the service by engaging with educational activities and developing patient pathways for safe discharge and remote monitoring.
  • To recognise opportunities within the service for further development and expansion, to facilitate and manage change within defined clinical and organisational boundaries.
  • To case manage a team of patients in partnership with the multi-professional team and to be a patient advocate when required.
  • To develop own nurse led clinics in conjunction with the Consultants and the outpatient department.
  • Education

  • To identify, assess and meet the educational needs of the patient and their family.
  • To work collaboratively on educational strategies which support IBD surgery services and service development.
  • To act as a preceptor / mentor to nurses wishing to undertake further studies in IBD or ileoanal pouch surgery.
  • To develop their own knowledge, skills and expertise to a higher academic standard, by time spent with colleagues and attending study days
  • To develop resource and teaching packs for use by patients and/or staff in the clinical and educational setting.
  • Research/Audit

  • To utilise up to date research / evidence to improve patient care and to disseminate this knowledge to all members of the multi-professional team
  • To incorporate nationally agreed standards and guidelines into clinical practice.
  • To be aware of and participate in current research trials being conducted within own specialist area and to be able to inform patients about these.
  • To audit the service on a regular basis so that patient care is continuously reviewed and improved, ensuring that documentation and record keeping is comprehensively maintained
