An exciting opportunity is available to join our Overnight Response Service which will provide an unplanned specialist advice line and responsive visiting service to palliative care patients and carers overnight.
This specific role will involve working closely as a team of 2 with a Clinical Nurse Specialist.
You will be required to answer some telephone calls, undertake home visits jointly with the specialist nurse, carry out supportive patient care and input details on to SystmOne alongside other housekeeping duties to support the efficient running of the service.
Access to a pool car will be available. Driving is a key component of the role.
The pilot service will be supported by access to specialist palliative medical advice as needed and the service will operate between, 22.00 and 08.00 hours 7 days a week.
You will be required to work some day shifts to maintain skills and compliance with training requirements.
To be considered you will need to be a highly motivated individual, have experience of providing palliative care and end of life care within the community setting. Possess empathetic communication and interpersonal skills.
