Job summary

An excitingopportunity has arisen for a dynamic nurse to join the Intermediate Care Teamin First Community Health and Care.

The IntermediateCare Team (ICT) are an admission avoidance and facilitating discharge service,who provide assessment, short-term care and rehabilitation in the patientshome. You will complete comprehensive assessments in the patients home andundertake treatments including IV medications, wound assessment, andmanagement. You will work closely with the multi-disciplinary team to promote independence of patients to ensure they can remain in their ownhome.

The candidate willbe able to adapt to change and work in a busy environment. Excellent communication skills are essentialto enable liaison with all our health and social care partners in the management of patients discharge from hospital and assessment in thecommunity.

Main duties of the job

You will also provide professional leadership for designated staff andwill have a role in the leadership and line management of the wider multidisciplinaryteam.

Due to the nature of the role it is essential that applicants are ableto drive, have a car suitable for business purposes and have a clean drivinglicence.

The service runsfrom 8am-8pm 7 days a week, 365 days a year, the post holder will be expectedto work on a rota basis. There are opportunities for both full time and parttime hours.
