An exciting opportunity is available to work in Community Nursing based at Hunter Street Health Centre in in the heart of Camden, North West London.

The service is based between Camden Town, Kentish Town, and Covent Garden, it operates from 08:30 to 17:00hrs seven days a week and you will also work alongside Complex Care Nurses, Occupational Therapists, Physiotherapists, pharmacy technicians and students.

Main duties of the job

The post holder will have responsibility for a caseload of around 80 patients with opportunities to oversee and manage their care. The post holder will be supported by and line-manage a well-established skilled team including Healthcare Assistants, Nurse Associates and Nurses from band 3 to band 6.

The post would suit an experienced Community Nurses who has undertaken prior caseload management and has a passion for providing excellent patient centred care. Support is available onsite from the Nurse Lead who oversees the area’s four micro teams and complex care.

As well as your integrated care colleagues, you will have access to and support from the wider expertise of our specialist teams including Evening and Overnight Nursing team, early supported discharge team, Rapid Response, Palliative Care, Tissue Viability, bladder & bowel, community stroke and neurology team, respiratory, heart failure and diabetes teams as well as the in-patient wards: who all aim to work very collaboratively. Your role will include working closely with our GP colleagues and practices.

Detailed job description and main responsibilities

Clinical Practice
Community Team Leads will: maintain close professional relationships and patient pathways with other teams within IPC (Rapid Response, D2A, Twilight/overnight, central booking; in-patient areas, LTCs etc.) GPs and other professionals and attend primary care practice and MDT meetings on behalf of the team as appropriate.

Manage and delegate as appropriate the full clinical range of adult services including supporting the wider IPC teams in the management of individuals with long term conditions and targeting services at vulnerable groups such as those in Care Homes. 


Provide clinical leadership line management and clinical supervision to clinicians from the same professional group in the team, ensuring high quality care delivery

Support other senior team members in delivering supervision and clinical advice and support to team members. 

Support and assist with budget management in the team ensuring the best use of financial resources remaining within the designated budget.

 Participate in all clinical governance, business, training and reflective practice meetings, leading these as delegated by the team leaders.

Contribute to the development and implementation of clinical policies and procedures based on evidence and recommendations arising from national policy on long term conditions. 


Work in partnership with older people, vulnerable adults, their carers and families making sure they are involved in decision making when planning their care.

 Attend regular management and senior team meetings as appropriate and deputise as necessary.
Identify vulnerable adults and support the provision of effective programmes of support that protect and promote their health and wellbeing, intervening appropriately when a person’s safety or welfare is at risk. 


Work closely with all members of the Primary Health Care Team ensuring that colleagues are aware of the services offered. 


Cover for the absences of colleagues across the localities as appropriate and as required 
by service needs. 


Contribute to the implementation of the Clinical Governance agenda including agreeing the 
outcomes for the Team, Clinical Audit and Quality Improvement.


Expert Practice
Be involved in the development of close professional relationships and attending primary 
care practice meetings and MDT’s.

Work at the interface between the team and statutory and voluntary organisations, to facilitate cohesive approaches to meeting the diverse needs of people in Camden.


Maintain efficient and effective communication both, verbal and written with clients, their relatives, colleagues and external stakeholders.


Keep accurate and contemporaneous records of all clinical interventions, multi agency liaison and supervisory activity with staff.


Comply with current data collation and protection systems, provide written reports as Undertake clinical activities directly related to the assessment, planning and evaluation of patient care.

To use own judgement to assess and interpret specialist acute or other patient interventions.


Be responsible for the delivery of health promotion programmes and other preventive services, including flu immunisation, ensuring access to vulnerable groups.


Carry out health needs assessment of individuals using the single assessment process where appropriate.

Take responsibility for prescribing as deemed professionally appropriate in line with the Non Medical Prescribing/Nursing Formulary and the Trust protocols for Non Medical Prescribers.


Identify opportunities for practice development through, research, and clinical audit.


Leadership and Management
To facilitate the data gathering and reporting of service activities.

To contribute to the recruitment and selection of staff for the team and to participate in induction and orientation programmes. This will include acting as preceptor for newly qualified staff.


To manage and co-ordinate the work of registered nurses and health care support assistants, monitoring and appraising their performance, offering support, development and training as appropriate.


To take responsibility for ensuring own mandatory training is up to date and those of the team members are similarly.


To be responsible for the day to day operational management of direct line reports including engaging them in annual appraisal and objective setting.


To support the Team Lead in the development and implementation of case management for those with long term conditions, thus facilitating the appropriate targeting of resources to those in most need.


To participate in the investigation of complaints.


To contribute to the successful implementation of organisational directives within the team and the achievement of annual team aims and objectives.


To assist with the management of the team budget, identifying opportunities for improving value for money within the team so that the overall aims and objectives of the service are achieved.


To improve access to services by ensuring that service provision and delivery is culturally appropriate for the targeted client group.


To monitor activity levels and inform management of any changes. Formulating and adjusting plans of a broad range of complex activities according to service needs. 


Service Quality
Facilitate the data gathering and reporting of service activities.

Manage and co-ordinate the work of registered nurses, physiotherapists occupational therapists, speech and language therapists and rehabilitation assistants and health care assistants, monitoring and appraising their performance, offering support, development and training as appropriate to your professional registration. 


Be responsible for the day to day clinical practice of your clinical team including engaging them in annual appraisal, monthly supervision and objective setting.

Plan and participate in the orientation, teaching and supervision of community staff and students.


Support, teach and assess the developing clinical skills of junior team members in response to the changing needs within the localities.

Promote evidence based practice that responds to local and national service standards, and to act as a resource to colleagues.

Education 
To plan and participate in the orientation, teaching and supervision of community staff and students.

To maintain continuous personal, professional development and clinical skills in order to respond to any changing population health needs in response to the national and local priorities, through audit and appraisals scheme.


To act in the capacity of practice assessor in the support of nursing students undertaking post graduate community nursing studies. 


Professional Conduct

To deliver care in accordance with the Trust policies, procedures and guidelines. 


To comply with the NMC Code of Professional Conduct and maintain valid professional registration. 

To be accountable for actions and any omissions relating to own clinical practice and advice given to members of the team.


To maintain own professional skills and knowledge base, by undertaking relevant education and 
training.


To take part in clinical practice reflective sessions with professionals advisors/designated nurse and annual objective setting/appraisal with line manager.


Research and Audit 
To be conversant with key national policy on long term conditions, admission avoidance, adult protection, National Service Frameworks, National Institute of Clinical Excellence (NICE) and other key national strategies.


To identify opportunities for practice development through, research, and clinical audit. 
To promote evidence based practice that responds to local and national service standards, and to act as a resource to colleagues.


To identify strength and weaknesses from practice and share information via team meetings and in 1:1 clinical feedback with staff. 


To facilitate practice based learning for all team members especially new staff and students. 


To participate in healthcare audit and research.


Safeguarding 
All staff have a duty to safeguard and promote the welfare of children and adults at risk of abuse. 


Staff should be aware of local safeguarding procedures and how to contact named professionals for 
advice and support as well as reporting any concerns.

Staff are supported with the supervision and training needed to recognise and act on welfare concerns and to respond to the needs of children and adults.

Staff have a responsibility to ensure they are up to date with any safeguarding training.
