You will be:
A qualified nurse with appropriate skills and experience working in the palliative / End of life arena
A well organised. Flexible professional
Compassionate to the needs of palliative and end of life care patients/carers
An understanding of the 4 organisations who make up the palliative care providers in this division
Used to working collaboratively as part of a multidisciplinary team
Have a wide range of IT skills and experience with use of Systmone
Excellent communication skills
Knowledge and skills of community nursing
Triaging skills to ensure the best outcome for our patients
Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)

Main duties of the job
The Community Palliative Care Specialist Nurse will join an established integrated Cancer, Non Cancer and Palliative Care team that works across organisational boundaries. The post holder will be a member of the Central London Community Healthcare NHS Trust, Specialist Palliative Care Service. The Nurse will be required to influence patient care by undertaking:
o Advanced Clinical Practiceo Training and Educationo Service Development/Leadershipo Audit
This role within an established team will support ongoing development of the post holder, giving them the opportunity to advance their palliative and end of life care skills alongside a multiprofessional is a broad summary of the role detailing key areas of operation and responsibilities - further detail will be provided in the Main Duties and Responsibilities section, below, which provides more granular detail.

Job descriptionJob responsibilities
Provide highly specialist palliative care advice and support to other generalist nurses, care agencies and health professionals including doctors and consultants, to ensure high quality individualised patient care.
Use expert skills in communication, clinical judgement and management within palliative care settings.
Use and develop specialist skills when dealing with verbal and physical aggression whilst working with patients and carers.
Function effectively as an autonomous practitioner working independently not directly supervised and as a member of a multidisciplinary team.
