Job summary

Specialist nursing and therapyroles are critical to the successful delivery of care within Accelerate. The expectation is that the ClinicalLead for Complex Wound Care will lever their passion for their speciality andtheir expert skills in the care and management of complex patients for thedelivery of excellent outcomes for their patients and commissioners. It is alsoanticipated that the post holder will bring better understanding to theprinciples of patient flow, clinical systems and their organisation and staffeffectiveness and efficiency. You will bring creative thinking andinnovation to complex issues, utilising expert skills, risk management andinsight in order to effectively manage a variety of situations borne out ofcomplex psychosocial, medical or organisational need. The abilityto grasp the principles of clinical leadership, understand partnership workingacross a health economy and demonstrate a solution driven mentality areessentials of this role.

Main duties of the job

The Clinical Lead is consideredthe expert and will take ownership for the operational delivery of the clinicalservice in the Centre of Excellence and Innovation; with an ability to take apopulation health view rather than a pure individual focus. The post holderwill lead their team to deliver excellent clinical care alongside themanagement of risk and successful deployment of the teams resources. This roleis designed to enable the post holder to take full end to end responsibilityfor the delivery of the outcomes and indicators for a defined contractualbusiness area. Clinical and operational delivery remains core to this role; thepost holder is accountable to the Director of Clinical services for themanagement of the key performance targets.

Working collaboratively with theDirector of Clinical Services for Wound Care to maintain high quality woundcare with a focus on extending reach to our patient cohort outside of ourcurrent geographical boundaries.

Job description

Job responsibilities

The Clinical Lead for Complex WoundCare (CEI) will provide expert skills and depth of knowledge to the populationwithin the CEI primarily in lower limbs but be knowledgeable in all areas ofwound assessment and management of differing aetiologies. This knowledge willbe the enabler to critique our wound care population and align to outreachprojects and quality improvements.

Specifically,this requires focus on:

Delivery of expert care to our population ofwound care patients within the CEI recognising different needs and skills ofmanaging patients with very complex needs

Responsible for the day-to-day resourcemanagement of the resources, both skills and time, in the team.

Lead on supporting and training of staff inrelation to the delivery of QI programmes and evaluations..

Delivery of expert care to our population ofwound care patients beyond the CEI recognising different needs and skills ofmanaging patients with complex needs through virtual pathways

Monitor monthly use of dressing spend within theCEI and ensure minimal wastage and cost effectiveness of products used

Networking and building relationships across localacute services such as dermatology, vascular and Haematology depts in order tosupport a seamless service to our patient population.

Highlight to the Patient Services Managerequipment for servicing/faults so that this can be dealt with in a timelymanner

Additional responsibilities focus on:

Analysis of footfall of patients via CEI inorder to stratify and identify those for repatriation back to the local area

Engagement in any related Academy programmes inthe UK as required

Decision making in partnership with the ClinicalLead for Community Wound Care on product evaluation across sites andformularies

Contribute to a multidisciplinary strategy ofcare and supported self-management plan that considers patients choices andabilities

Be creative in looking at ways that patients canmonitor behaviour change and meet specific goals

Maintain clear records using the Acceleraterecord keeping frameworks and ensure this within their team

Develop the nursing or therapy roles within yourservice area by

oEstablishing work boundaries and being clearabout role impact

oData collection which is specific to the rolewith an attention to agreed outcomes

oIdentification of themes and trends that willcontribute to future projects

oParticipation at key Accelerate events such asAway Days or Quarterly Reviews

This role will be embedded withina multidisciplinary clinical team. As such there will be an expectation thatthe inherent knowledge and skills will be used to:-

Facilitate reflection for clinical andorganisational growth; this will be within the clinical team and on occasionsthe Business support team

Contribute to patient engagement and empowermentinitiatives and projects. This will include tracking peoples journeys andcapturing key themes concerning experience and learning

Write articles and present at national conferencesas part of Accelerates strategy for reaching out into the wider healthcaresetting and for your professional growth

Qualifications

Essential

  • First Level Registered Nurse
  • Current NMC Registration
  • University degree demonstrating specialist knowledge and/or Masters
  • Wound Management module
  • Desirable

  • Chronic oedema training
  • Teaching and assessing course
  • Prescribing course
  • Skills

    Essential

  • Delivery of Clinical Leadership
  • Able to work with minimal supervision in assessing, planning, implementing, and evaluating care for patients
  • Experience of supervising and mentoring other health professionals
  • Experience of managing conflict within teams and patient delivery

  • Experience of implementing root cause analysis
