We are currently seeking a compassionate, skilled, and dedicated Children's and YPs Continuing Care Nurse Assessor to join us in our mission to make a meaningful difference in the lives of those we serve.
Position: Children's and YPs Continuing Care Nurse Assessor
Location: East Midlands
Type of Employment: Contract, outside of IR35
Day rate: £335pd
Key Responsibilities:
Carry out detailed assessments for children and YPs to determine eligibility for continuing care in line with national frameworks.
Develop, implement, and review comprehensive, personalized care plans in collaboration with the child and, their family, and multidisciplinary teams.
Advocate for the child or person's healthcare needs, ensuring their voice and their family's wishes are central to care planning and delivery.
Collaborate with healthcare professionals, educational institutions, social services, and other agencies to provide holistic support.
Monitor and evaluate the effectiveness of care plans, adapting to changing needs as necessary.
Requirements:
Registered Nurse (Children's Nurse) or relevant healthcare professional with current registration and experience in children's nursing or pediatric care.
Demonstrated experience in assessing and planning care for children and persons with complex health needs.
Good communication and interpersonal skills, with an ability to work sensitively with children, persons, and their families.
Excellent organizational skills with the ability to manage multiple priorities.
A commitment to child-centered care and an advocate for the rights and needs of children and YPs.
