A day in the life of a Assist to Discharge Support Worker will include:

Flexible working and fast paced changes.

Playing a vital role in providing practical and emotional assistance to individuals in their own homes assessing their needs

Working closely with people, their families and other professionals to create care plans tailored to their needs and goals.

Assisting and enabling individuals with activities of daily living, including bathing, dressing, meal preparation medication management, and mobility support.

Collaborating with healthcare professionals, social workers, and other team members to ensure a comprehensive and coordinated approach to care.

Monitoring and evaluate individuals progress, adjusting care plans as needed to promote optimal recovery and well-being.

Maintaining accurate and thorough documentation of interactions, care plans, and progress.

To be a successful Assist to Discharge Support Worker you'll need:

Previous experience in a similar care, preferably within a healthcare or social care setting.

Compassionate and empathetic nature, with a strong desire to make a positive impact on individuals’ lives.

Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.

Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.

Ability to work independently, demonstrating initiative and reliability.

Flexibility to work various shifts, including evenings, weekends, and bank holidays.

A full driving license and access to a vehicle is preferred but not mandatory.
