The Single Point of Access team (SPA) was established in 2009, to provide timely advice, information and triage of referrals received for Adult Social Care and Health Services. Working within strengths based, dynamic service delivery model, we are now ready to continue to develop the service, to ensure we are achieving excellent outcomes for residents of North Somerset.
SPA is a diverse team comprising of Health and Social Care professionals and support colleagues. We are now looking for a confident Adult Social Care Worker, with good time management skills, who can work in a quick moving environment to make sound evidenced based decisions to meet individual’s needs, efficiently and effectively.
About the role
This is a challenging role where you will screen incoming referrals with the view to immediate resolution or triage on for further assessment. The role will provide you with the opportunity to work within an integrated health and social care team, whilst developing your skills and experience, you will be supported by Senior Practitioners and you will have access to appropriate training and development for the role. You will be joining the team at an exciting time in its development and it is hoped that you will make a positive contribution to the expansion of the Triage Service within the SPA.
Various hours are currently available up to full time.
