The Adults Future Options service is an innovative and vibrant service who work with adults with a disability. The fundamental aim of the service is to empower them to have more choice and control over their lives, to live independently in the community or work towards this as a future goal.

The teams work with adults who have care and support provided in their own homes, in supported living schemes, and in residential and nursing care homes in Sheffield and across the Country. They have already established close working relationships with a variety of primary health colleagues, health and therapy professionals in community learning disability teams, advocacy, housing, communities, and voluntary sector providers who are supporting people in their local area. The service also works with parents, carers, and families to ensure a person-centred approach is always maintained throughout the assessment process and to allow co-production of care and support plans.

We are looking for motivated social workers and experienced social workers who have experience of working with vulnerable adults with a disability and complex needs. A key aspect of the role is complex case management, assessment, and support and to understand quality, quantity, and value of care and support provision therefore experience in working with providers to promote best value and practice and, respond to and manage safeguarding situations as they arise is an essential part of the role.

You will have a working knowledge of promoting independence in a least restrictive environment under current national frameworks and legislation and so an extensive knowledge of the Care Act, Mental Capacity Act, and Deprivation of Liberty Safeguards is required to work with providers and the Court of Protection to achieve best outcomes. Knowledge and experience of the Continuing Health Care Framework is also a requirement due to the complex needs of the individuals’ we work with.
