About this role

This position assists the Manager Health Promotion to coordinate, plan, implement and evaluate health promotion interventions with Aboriginal and non-Aboriginal populations for the primary prevention of chronic disease within the North Metropolitan Health Service (NMHS) catchment area; assists to determine the strategic priorities for health promotion for the NMHS. Collaborates to implement agreed programs and takes a lead role to support and guide teams formed to implement those programs; monitors, evaluates and reports on activities and outcomes; consults with government departments, non-government organisations, Aboriginal Community Controlled Organisations and the community to ensure intersectoral support for health promotion initiatives.

The works across the NMHS catchment area and aims to keep the population healthy by preventing chronic diseases ( cardiovascular disease, type 2 diabetes, respiratory disease and some cancers) and injury. The Service has a population-wide approach and focuses on the main lifestyle risk factors causing the greatest burden of disease: smoking; poor diet; harmful alcohol use; overweight and obesity; and physical inactivity. Health Promotion professionals identify the causes of these risk factors and work collaboratively with local organisations and community to find ways to address them.

In this position you will lead the Priority Communities Program and collaborate with local service providers and community in priority localities to develop tailored interventions that support healthy behaviours and create safer environments. 

This position reports to the Manager Health Promotion.
